Mechanism of Action

Empagliflozin inhibits the Sodium-Glucose Co-transporter 2 (SGLT-2) in the kidneys, preventing glucose reabsorption into the bloodstream and increasing its excretion through urine, thereby lowering blood glucose levels.
Metformin lowers blood glucose by reducing hepatic glucose production and decreasing intestinal absorption of glucose. It also enhances insulin sensitivity in peripheral tissues.

Precautions

Warnings & Precautions

Consult your doctor or pharmacist before taking Paglimet if you:

  • Have urinary problems or kidney disease
  • Have liver disease
  • Have type 1 diabetes
  • Experience pain, redness, or swelling in the genital area or between the genitals and anus
  • Have a vitamin B12 deficiency
  • Are scheduled to receive iodinated contrast agents
  • Have diabetic skin complications such as diabetic foot ulcers
  • Have diarrhea, fever, or cannot eat or drink adequately
  • Are older than 75 years
  • Experience symptoms such as nausea, vomiting, hypothermia, bradycardia, respiratory distress, or muscle cramps

Pregnancy & Breastfeeding

  • If you are planning to become pregnant, are pregnant, or are breastfeeding, consult your doctor or pharmacist before using this medication.
  • Use of Paglimet during pregnancy is not recommended as its safety has not been fully established.
  • While small amounts of metformin may pass into breast milk, it is not known whether empagliflozin does. Therefore, Paglimet should be avoided during breastfeeding.
Drug Interactions

Inform your doctor or pharmacist if you are taking, have recently taken, or may take any of the following while using Paglimet:

  • Alcohol
  • Iodinated contrast media
  • Insulin
  • Sulfonylureas (e.g., glibenclamide, gliclazide)
  • Cimetidine
  • Ranolazine
Side Effects

Like all medicines, Paglimet may cause side effects, although not everybody experiences them.

Side effect with 1-10% frequency:

  • Hypoglycemia (low blood sugar levels), especially when used with insulin or sulfonylureas

If You Take More Paglimet Than You Should
If you take more Paglimet than prescribed, it may lead to lactic acidosis, a serious condition.
Symptoms include weakness, vomiting, stomach pain, muscle cramps, severe fatigue, and difficulty breathing. Other signs may include lowered body temperature and slow heartbeat.
Lactic acidosis may lead to coma and requires immediate medical attention. Stop taking the medication and contact your doctor or the nearest hospital right away.

If You Forget to Take Paglimet
Take the missed dose as soon as you remember. If it is close to your next dose, skip the missed dose and take only the next one.
Do not take a double dose to make up for a missed one.

Contraindications

Do not use Paglimet if you:

  • Are allergic to empagliflozin, metformin, or any other ingredients in the product
  • Have metabolic acidosis or diabetic ketoacidosis (characterized by high blood glucose, nausea, vomiting, hypothermia, bradycardia, respiratory problems, and muscle cramps)
  • Have severe renal impairment or are undergoing dialysis
Storage
  • Keep out of the sight and reach of children
  • Do not use after the expiry date printed on the packaging (after “Exp. Date”); the expiry date refers to the last day of the indicated month.
  • Store below 30°C, away from moisture and light
  • Keep the product in its original packaging until use

  • Immediate-release tablets are supplied in bottles containing 60 tablets.
  • Extended-release tablets are supplied in bottles containing 30 tablets.
