Avanomet

Mechanism of Action

Empagliflozin inhibits the Sodium-Glucose Co-transporter 2 (SGLT-2) in the kidneys, reducing glucose reabsorption into the bloodstream and increasing its excretion through urine, thereby lowering blood glucose levels.
Linagliptin inhibits the Dipeptidyl Peptidase-4 (DPP-4) enzyme, prolonging the activity of incretin hormones. This results in enhanced insulin secretion, suppressed glucagon release, and ultimately improved glycemic control.
Metformin lowers blood glucose by reducing hepatic glucose production, decreasing intestinal glucose absorption, and improving insulin sensitivity.

This medication is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.

Dosage Instructions
  • The recommended dose of Avanomet is once daily. Your physician will determine the appropriate dosage for you.
  • Take Avanomet with your morning meal.
Precautions

Warnings & Precautions

Consult your doctor or pharmacist before using Avanomet if you:

  • Experience skin symptoms such as blistering, burning, or itching.
  • Have heart conditions such as heart failure.
  • Have kidney impairment.
  • Have symptoms like pain, redness, or swelling between the genitals and anus.
  • Have a vitamin B12 deficiency.
  • Have diabetic skin complications such as foot ulcers.
  • Are experiencing diarrhea, fever, or you are unable to eat or drink.
  • Are taking other antidiabetic medications.
  • Have symptoms such as nausea, vomiting, hypothermia, bradycardia, respiratory difficulty, or muscle cramps.

Pregnancy & Breastfeeding

  • If you are pregnant, may be pregnant, planning to become pregnant, or breastfeeding, consult your doctor or pharmacist before using this medicine.
  • Avanomet is not recommended during the second and third trimesters of pregnancy, as its safety for the fetus has not been fully established.
  • Metformin is excreted in small amounts in breast milk. It is unknown whether linagliptin or empagliflozin are also excreted. Therefore, use during lactation is not recommended.
Drug Interactions

Inform your doctor or pharmacist if you are currently using, have recently used, or may use any of the following medications:

  • Alcohol
  • Iodinated contrast agents
  • Insulin
  • Sulfonylureas (e.g., glibenclamide, gliclazide)
  • Cimetidine
  • Ranolazine
  • Anticonvulsants such as carbamazepine, phenobarbital, phenytoin
  • Rifampin
Side Effects

Like all medicines, Avanomet may cause side effects, although not everyone gets them.
Side effects occurring in 1% to 10% of patients include:

  • Upper respiratory tract infection and nasopharyngitis (inflammation of the nose and throat)
  • Diarrhea, constipation, gastroenteritis (viral infection of the stomach and small bowel)
  • Headache
  • Urinary tract infection

If You Take More Avanomet Than You Should
If you take more than the prescribed dose of Avanomet, you may develop lactic acidosis. Symptoms include weakness, vomiting, stomach pain, muscle cramps, extreme fatigue, and breathing difficulties. Other signs include low body temperature and slow heart rate.
Discontinue use immediately and seek emergency medical attention, as lactic acidosis may lead to coma.

If You Forget to Take Avanomet
If you forget a dose of Avanomet, take it as soon as you remember. If it is close to your next scheduled dose, skip the missed one and take only the next dose. Never double the dose to make up for the missed one.

Contraindications

Do not use Avanomet if:

  • You are allergic to metformin, linagliptin, empagliflozin, or any of the other ingredients in the formulation.
  • You have metabolic acidosis or diabetic ketoacidosis (symptoms may include high blood glucose, nausea, vomiting, low body temperature, and heart rate, breathing problems, and muscle cramps).
  • You have severe kidney impairment or are undergoing dialysis.
Storage
  • Keep out of sight and reach of children.
  • Do not use after the expiry date stated on the packaging (Exp. Date:), which refers to the last day of that month.
  • Store below 30°C, away from light and moisture.
  • Keep in the original packaging until use.
