Mechanism of Action

Linagliptin inhibits the Dipeptidyl Peptidase-4 (DPP-4) enzyme, thereby increasing the half-life of incretin hormones, resulting in enhanced insulin secretion, reduced glucagon secretion, and subsequently lowering blood glucose levels.
Metformin decreases hepatic glucose production and intestinal glucose absorption, and increases insulin sensitivity, thereby reducing blood glucose levels.

Indications (Uses)

This medication is used to improve glycemic control in adult patients with type 2 diabetes mellitus, alongside diet and exercise.

Dosage Instructions
  • The recommended dosage of Avamet is two tablets daily. However, the exact dosage may vary depending on the patient’s condition and the dosage of other antidiabetic medications being used. Your physician will determine the appropriate dosage for you.
  • Take Avamet tablets with food.
Precautions

Warnings & Precautions

Consult your doctor or pharmacist before taking Avamet in the following situations:

  • You experience skin reactions such as blisters, burning, or itching.
  • You have diabetic skin lesions such as diabetic foot ulcers.
  • You have diarrhea or fever, or if you are unable to eat or drink.
  • You are using other antidiabetic medications.
  • You experience nausea and vomiting, low body temperature and a slow heart rate, breathing difficulties, or muscle cramps.

Pregnancy & Breastfeeding

  • Consult your doctor or pharmacist if you are planning to become pregnant, suspect you may be pregnant, or are breastfeeding.
  • Use of Avamet during pregnancy is not recommended, as its safety for the fetus has not been fully established.
  • Metformin passes into breast milk in small amounts. It is unknown whether linagliptin passes into breast milk. If you intend to breastfeed, consult your doctor before using this medication.
Drug Interactions

Inform your doctor or pharmacist if you are currently taking, have recently taken, or might take any of the following medications while using Avamet:

  • Alcohol
  • Iodinated contrast media
  • Insulin
  • Sulfonylureas (e.g., glibenclamide, gliclazide)
  • Cimetidine
  • Ranolazine
  • Antiepileptic drugs such as carbamazepine, phenobarbital, or phenytoin
  • Rifampin (an antibiotic used to treat tuberculosis)
Side Effects

Like all medicines, Avamet may cause side effects, although not everyone experiences them.
Side effects occurring in 1% to 10% of patients include:

  •  Upper respiratory tract infections and nasopharyngitis (inflammation of the nose and throat)
  • Diarrhea

If You Take More Avamet Than You Should
If you take more Avamet than prescribed, you may develop lactic acidosis. Symptoms include weakness, vomiting, stomach pain, muscle cramps, severe fatigue, and breathing difficulties. Other signs include low body temperature and slow heart rate. This condition may lead to coma and requires urgent medical attention. Discontinue use and immediately contact your doctor or the nearest hospital.

If You Forget to Take Avamet
Take the missed dose as soon as you remember. If it is close to the time of your next dose (less than 12 hours), skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.

Contraindications

Do not take Avamet if:

  • You are allergic to metformin, linagliptin, or any of the other ingredients in the formulation.
  • You have metabolic acidosis or diabetic ketoacidosis (increased ketones in blood or urine), with symptoms such as high blood sugar, nausea, vomiting, low body temperature and slow heart rate, breathing difficulties, and muscle cramps
  • You have severe renal impairment.
Storage
  • Keep out of the sight and reach of children.
  • Do not use the product after the expiry date printed on the packaging (after “Exp. Date:”), which refers to the last day of that month.
  • Store in a dry place, away from light and moisture, at a temperature below 30°C.
  • Keep the medication in its original packaging until use.
